Web Form Information
When you submit information to Patriots Plan to make a payment or open an account, we may collect personally identifiable information from you that may include your name, tax id, address, email address, phone number, and credit card number. We also collect personally identifiable information from you if you participate in our promotions, such as contests and sweepstakes, take our surveys, use our services, contact us about a transaction, or email, call or otherwise communicate with us. We do not knowingly collect personally identifiable information from persons under the age of 18. If in any case that we found out that information for a person under the age of 18 has been submitted to us, we will delete it within a reasonable timeframe.
We may also collect, from you, the following personal information about your contacts (name and address). When you provide us with personal information about your contacts we will only use this information for the specific reason for which it is provided.
